I have an at fault accident on my record that drove up my rates. Due to changing situation I may be moving to a new state while changing my name and auto policy.
This is not me trying to intentionally game the system, but will this record follow me to a different state under a different name if the old insurance company never had my new name?

If you change your name on your drivers license and exchange it to the new state it will follow you.
When you get a policy you are asked if you have had claims. Not being honest is insurance fraud.
