This is a 3 minutes presentation of my PhD research focusing on incident investigation process and practices of the Ghanaian mining industry. Below is a short description of the research

    The Ghanaian mining industry continues to be associated with incident rate exceeds that of major gold producing countries. The industry’s 10-year (2004-2013) fatality frequency rate (0.0711) significantly exceeds that of Australia (0.0279) and USA (0.0569). This high incident rate is attributed to the industry failure to learn from its own internal experience and the experience of others. By identifying the causes of this failure, the industry’s learning capacity could be improved. This research evaluates how incident investigations are actually conducted through a case study methodology. The research extends upon existing learning from incidents literature by providing empirical knowledge on how learning from incidents occurs, the constraints impeding effective learning from incidents, and how to assess the effectiveness of practical investigations. A novel approach, which extends the application of the maturity model concepts to incident investigations, to provide a practical tool that industry can use to self-assess their incident investigation and identify areas for improvement was developed
