Insurance pre-determination, what exactly is it?

My dentist sent an insurance pre-determination to my insurance company. When such determination is sent, what documents were exactly sent out?

Is it a PDF? A fax? Or similar to tax e-filing that not human readable string of encoded numbers in certain protocol?

I ask because I wonder if I can get a copy of the pre-determination.
