Insurence denied it. PCP and surgeon said it IS necessary, and the psychiatrist approved it. BMI was over 60, sleep apnia and high blood pressure are issues. How on earth is this insurence denying it? What do I do?

Your docs are going to need to document more obesity-related problems than that. If they are bariatric surgery specialists, they know what to document. They will also need to document which diet plans you have tried and failed at. If you have tried hypnosis, behavior modification, or other treatments which have failed, those need to be documented.
