I just found out my old Citibank/GTC account was canceled due to a small payment discrepancy during my last PCS. Apparently I need to re apply for the GTC through my APC.
It’s been years since I have applied, but I’m afraid my credit score has taken some hits. I know you can fill out a DD Form 2883 instead of running a credit check, but my APC is telling me that she has never seen that form and is unaware of how to fill out her portion.
If I apply for the GTC and submit it without the DD Form 2883 will I get denied? If my credit score is below the threshold will it get accepted and automatically get put as a restricted account?
Follow up question does anyone have an example of a properly filled out DD form 2883?
If anyone could help me I would really appreciate it.

APC no longer submit reinstatement applications as of 6/17/2024. Go on citimanager.com and apply for a new card
