I think someone used my vehicle info to commit fraud

Recently I requested a quote for a new insurance policy. When I was about to purchase a notification popped up and asked if this unknown person (listed as the driver of my vehicle ) lived in my household when my vehicle was purportedly involved in an at fault accident. A few months ago my current insurance company called me more than once asking who this person was and their relationship to me. I told them I had never heard that name in my life and I had no idea who it was. Then a few weeks later they called again and asked the same thing and I again told them I had no idea and asked what it was about and why they kept calling me about this. They said they would get back in contact with me and I never heard anything else about it. What do I do about this? It was reported by the motor vehicle department. How does something like this happen?
